Integritetspolicy
Denna policy förklarar hur vår webbplats tandlakargruppentaby.se samlar in och behandlar dina uppgifter, vilka val och möjligheter du har, och hur du kan utöva dina rättigheter. Denna policy riktar sig till dig som besöker vår webbplats och skickar epost till oss, både via våra kontaktformulär på hemsidan samt via vår epostadress info@tandlakargruppentaby.se
Följande avsnitt förklarar vilka typer av uppgifter som vi kan komma att behandla om dig och för vilka syften. Vi förklarar också hur uppgifterna samlas in och hur länge uppgifterna sparas.
Tidsbokning och kontakt
Som patient hos oss har du möjlighet att begära tidsbokning via kontaktformuläret på vår webbplats eller per telefon. För att tillhandahålla dig denna tjänst krävs ditt namn, epost och telefonnummer. Behandlingen av dina uppgifter är nödvändig för att kunna fullgöra en tidsbokning och för att fullgöra rättsliga förpliktelser som åvilar oss i egenskap av vårdgivare. Kontaktuppgifterna sparas i vårt patientjournalsystem i enlighet med bestämmelserna i Patientdatalagen.
Epost
Epost du skickar till oss raderar vi så snart vi kan efter ditt ärende är avklarat. Om du önskar att ditt epostmeddelande raderas omgående så meddela oss detta via kontaktuppgifterna du finner längst ned i denna policy.
Patientuppgifter
Varje gång du kontaktar oss registrerar vi personuppgifter om dig. Personuppgifterna behövs för att kunna ge en god, säker och trygg tandvård. Personuppgifterna används också för att följa upp vår verksamhet och för att få en bild av våra patienters tandhälsa. De två lagar som reglerar hanteringen av personuppgifter inom vår verksamhet är Patientdatalagen och Dataskyddsförordningen.
Journalföring
Vi är skyldiga att föra en patientjournal avseende varje patient. Patientjournalen kan exempelvis innehålla personuppgifter som bakgrunden till vården, uppgift om ställd diagnos, utförda och planerade åtgärder och uppgift om när och vem som har gjort anteckningen i journalen. Dina personuppgifter behövs och behandlas för att kunna ge en god och säker vård, samt för verksamhets- och kvalitetsuppföljning. En del av personuppgifterna måste dessutom rapporteras till nationella hälsodataregister och Försäkringskassan. Vi har rätt att registrera dina personuppgifter i patientjournalen och hälso- och sjukvårdsregister även om du inte gett oss ditt samtycke. Dina uppgifter sparas i vårt patientjournalsystem i enlighet med bestämmelserna i Patientdatalagen.
Personuppgifter
Det finns klara regler i Patientdatalagen och Dataskyddsförordningen för hur vi ska bevara dina personuppgifter. Enligt dessa regelverk ska exempelvis personuppgifter i en journalhandling bevaras minst tio år efter det att den sista personuppgiften fördes in i handlingen.
Tillgång
Inom tandvården råder sträng sekretess för uppgifter om patienters hälsotillstånd eller andra personliga förhållanden. Vi kommer därför inte att lämna ut dina personuppgifter om vi inte är skyldiga att göra det till följd av lagar och föreskrifter (t.ex. till Försäkringskassan, Socialstyrelsen och Smittskyddsinstitutet), eller om vi har fått ditt uttryckliga samtycke till sådant utlämnande. Detta utesluter inte att vi kan använda leverantörer som behandlar dina personuppgifter för vår räkning i enlighet med skriftliga avtal och våra instruktioner. Leverantören som får tillgång till dina personuppgifter ges inte någon rätt att använda dina personuppgifter för andra syften än de syften som anges enligt denna patientinformation.
Ta del av patientjournalen
Som patient har du rätt att ta del av uppgifter som finns i din patientjournal. Du har även rätt att få en kopia av hela eller delar av patientjournalen.
Få dina personuppgifter rättade eller raderade
Om du har en avvikande mening i förhållande till det som står i din patientjournal, eller anser att en uppgift där är oriktig, har du rätt att få detta infört i din patientjournal. För att få en uppgift borttagen i din patientjournal måste du vända dig till Inspektionen för vård och omsorg (IVO) på www.ivo.se för att få detta prövat.
Återkalla ditt samtycke
Om du har lämnat ditt samtycke för behandling av dina personuppgifter har du rätt att när som helst återkalla detta samtycke. Du kan återkalla ditt samtycke genom att kontakta oss enligt kontaktuppgifterna nedan. Se dock ovan under rubriken ”Personuppgifter”.
Klagomål och skadestånd
Du har rätt att när som helst inge ett klagomål avseende vår behandling av dina personuppgifter till oss eller till Datainspektionen.

Patient information
Every time you contact us, we register personal information about you. The personal information is necessary to provide good, safe, and secure dental care. The personal information is also used to follow up on our operations and to get an overview of our patients' dental health. The two laws that regulate the handling of personal information within our operations are the Patient Data Act and the Data Protection Regulation.
Journalizing
We are required to maintain a patient record for each patient. The patient record may contain personal information such as the background of the care, details of the diagnosis, actions taken and planned, and information about when and who made the notes in the record. Your personal information is necessary and processed to provide good and safe care, as well as for operational and quality follow-up. Some of the personal information must also be reported to national health data registers and the Social Insurance Agency. We have the right to register your personal information in the patient record and in health and medical care registers even if you have not given us your consent. Your information is stored in our patient record system in accordance with the provisions of the Patient Data Act.
Personal data
There are clear rules in the Patient Data Act and the Data Protection Regulation regarding how we should preserve your personal data. According to these regulations, for example, personal data in a medical record must be kept for at least ten years after the last personal information was entered into the record.
Access
In dental care, strict confidentiality applies to information about patients' health conditions or other personal circumstances. Therefore, we will not disclose your personal data unless we are required to do so by laws and regulations (e.g., to the Social Insurance Agency, the National Board of Health and Welfare, and the Public Health Agency), or if we have received your explicit consent for such disclosure. To have an entry removed from your patient journal, you must contact the Health and Social Care Inspectorate (IVO) at www.ivo.se to have this assessed.
Revoke your consent
If you have given your consent for the processing of your personal data, you have the right to withdraw this consent at any time. You can withdraw your consent by contacting us using the contact details below. Please see above under the heading 'Personal Data'.
Complaints and damages
You have the right to file a complaint regarding our processing of your personal data with us or with the Data Inspection Authority at any time.
